Abstract
Objective: To evaluate whether structured interventions aimed at improving patient-professional communication, compared with usual care, enhance satisfaction, engagement, communication quality and clinical/process outcomes. Methods: Systematic review performed in accordance with PRISMA 2020. PubMed was searched (30 Mar 2025) using the MeSH terms “Physician-Patient Relations” AND “Communication”, with filters for randomized controlled trials, free full text and the last five years. Eighteen studies were included (12.477 patients; 1.059 professionals). Two reviewers independently carried out data extraction and risk-of-bias assessment (RoB 2). Results: Question Prompt Lists, conversation guides, electronic nudges, training programmes and chatbots increased satisfaction and engagement by up to 54 %, improved perceived empathy and, in four studies, reduced pain, blood pressure or HbA1c. Interventions lacking intensive human contact had limited effects. Heterogeneity precluded meta-analysis. Conclusions: Structured communication acts as a therapeutic adjunct with demonstrable clinical value. Effective implementation demands cultural tailoring, pre-visit preparation, and a blend of digital modules with in-person feedback. Future research should standardise outcomes, include vulnerable populations and examine cost-effectiveness.
