Abstract

This study conducted a systematic review and meta-analysis to assess risk factors associated with COVID-19 morbidity and mortality in individuals aged 65 and over, stratified by geographic regions (Asia, Europe, Americas, and Africa). Thirty-three observational studies encompassing 13,498 participants were included. Findings show that admission hypoxemia (OR 4.88), thrombocytopenia (OR 4.19), and elevated serum urea (OR 3.12) are the strongest predictors of adverse outcomes. Composite markers—SOFA score, D-dimer, and C-reactive protein—also demonstrated consistent associations with mortality. Serum albumin had a protective effect, while lymphopenia and ferritin were poor predictors. Regional heterogeneity highlights the impact of local healthcare and socioeconomic conditions. We recommend integrating these parameters into region-adapted clinical protocols and conducting prospective multicenter studies that incorporate socioeconomic determinants and advanced predictive modeling.
