Resumo
Este estudo realizou revisão sistemática e metanálise para avaliar fatores de risco associados à morbimortalidade de COVID-19 em indivíduos com 65 anos ou mais, estratificados por regiões geográficas (Ásia, Europa, Américas e África). Foram incluídos 33 estudos observacionais totalizando 13.498 participantes. Os achados indicam que hipoxemia na admissão (OR 4,88), trombocitopenia (OR 4,19) e elevação de ureia (OR 3,12) são os preditores mais fortes de desfechos adversos. Marcadores compostos—escore SOFA, D-dímero e proteína C-reativa—também mostraram associação consistente com mortalidade. A albumina exerceu efeito protetor, enquanto linfopenia e ferritina foram pouco preditivas. A heterogeneidade regional reforça a influência de condições locais na gravidade da doença. Recomenda-se a incorporação desses parâmetros em protocolos clínicos adaptados à realidade de cada região, bem como a realização de estudos prospectivos multicêntricos que incluam determinantes socioeconômicos e modelagem preditiva avançada.
